Why Do Testing?

Testing may be required for your company. It helps to insure the safety of not only your workers, but also the public. Ensuring that your employees are tested can lead to fewer workers compensation claims and better employee attendance and morale.

General Form Instructions

A fillable PDF form has been downloaded to your computer. In some cases, you may need to save the file before you can fill it it. You must type in all applicable fields, then print the form. You can also just print the form. SIGN the printed form, and mail to the address provided thereupon.

Public Utilities Commission (PUC):

PUC regulations require that you must comply if:

  • All charter-party carrier applicants (new and renewal) who propose to employ any driver who will operate a vehicle having a seating capacity of 15 or less passengers including the driver
Exemptions are allowed for your driver if:
  • He/she is in a current program, with your company, which is mandated by the Federal Motor Carrier Safety Administration (FMCSA). If such time your driver is removed from the FMCSA program they must be immediately enrolled in your PUC program.
PUC programs mimic the FMCSA programs almost exactly. There are a few exceptions to the rules which must be complied with to keep your company in compliance. Our full service PUC drug and alcohol testing programs include employee drug testing, employee drug training as well as alcohol testing and training. Our drug training program also covers supervisor drug training, employee and supervisor alcohol training, handouts, posters, random testing selections and complete support. California Highway Patrol (CHP) - Rotation Tow Operations (CSAT):

CHP regulations require that you must comply if:

  • Your tow drivers are involved with CHP rotation tow operations
Exemptions are allowed for your driver if:
  • He/she is in a current program, with your company, which is mandated by the Federal Motor Carrier Safety Administration (FMCSA). If such time your driver is removed from the FMCSA program they must be immediately enrolled in your CSAT program.
CSAT programs mimic the FMCSA programs almost exactly. There are a few exceptions to the rules which must be complied with to keep your company in compliance.
